“Charlie” shared with his Housing Trust SASH Coordinator that he has begun looking at nursing homes because he feels that he is too much of a “burden”, particularly on his daughter, due to his propensity of falling (two recent falls he injured his head and the ambulance took him to the ED) and the frequent doctor’s appointments he must attend. The SASH Coordinator and Wellness Nurse spoke to him and learned that he wanted to stay home and not move. He enjoyed his independence, his home and his neighbors. He was referred to two volunteer driving organizations that could drive him to his appointments, lessening the “burden” of driving on his daughter and was given specific ideas on how to immediately decrease his chance of falling, which he agreed to implement. He was also referred to “Options Counseling” through the Council on Aging, to discuss other supports and insurances.
