SASH Coordinator – Support for Seniors/Adults with Disabilities

Cornerstone Housing Partners (Housing Trust of Rutland County, NeighborWorks of Western Vermont & Shires Housing) a nonprofit organization providing housing and services in Rutland County, is seeking a SASH Coordinator(SC).

The SASH Coordinator (Support and Services at Home) primary function is to build relationships and develop a thorough knowledge of the needs of each participant. The SASH Coordinator works directly with senior citizens and adults with disabilities, helping them remain safely in their home. The SASH Coordinator (SC) convenes the on-site SASH team on a regular basis to coordinate care and services to meet the needs of SASH participants. The SC identifies and proactively responds to the underlying causes of any resident or community problems and/or issues and involves his/her Supervisor as needed.

Skills

  • Knowledge of area resources and programs available to seniors and adults with disabilities, including transportation, health services, and recreational activities is essential.
  • Demonstrated excellence in verbal and written communication is required.
  • Must have knowledge of and an appreciation of the philosophy of a person’s choice to age at home.
  • Must possess a valid driver’s license.
  • Working knowledge of Microsoft Office and ability to learn program software.

Responsibilities

  • The SASH Coordinator (SC) convenes the on-site SASH team on a regular basis to coordinate care and services to meet the needs of SASH participants.
  • The SC identifies and proactively responds to the underlying causes of any resident or community problems and/or issues and involves their supervisor as needed.
  • The SC organizes and facilitates resident meetings monthly and serves as key on-site contact for issues relating to housekeeping, noise, site disturbances, etc.
  • The SC issues lease violations and warnings to residents and works with their supervisor on enforcement of the lease and resident handbook.

Qualifications

A Bachelor’s Degree in Social Work is preferred or equivalent combination of background and experience. Previous experience coordinating services specifically for the senior or adult with disability client population is also preferred.

Hours and Location

Part-time (20 hours per week), Monday-Friday. Office located in downtown Rutland, VT-travel to multiple sites throughout Rutland County (Benson, Fair Haven and Poultney) to meet with residents.

Compensation

Compensation is $19-$21 per hour plus mileage reimbursement.
